About DDHS

DDHS is a Perth-based NDIS provider supporting people living with disability and dementia throughout the Perth Metropolitan (primarily Joondalup to Baldivis).

While we have a particular passion for supporting people living with dementia, we proudly support participants of all ages and disabilities. We believe in delivering personalised support that builds confidence, promotes independence, and creates meaningful connections for every participant and their family.

About the Role

Every participant is unique, which means no two days are ever the same.

As a Support Worker with DDHS, you'll have the opportunity to build genuine relationships while supporting participants to live meaningful and independent lives.

Your responsibilities may include:

  • Personal care and daily living support
  • Community access and social participation
  • Transport to appointments and activities
  • Meal preparation and household assistance
  • Building confidence and independence
  • Supporting participants to achieve their individual goals
  • Providing companionship and emotional support
  • Working collaboratively with families, Support Coordinators and allied health professionals
  • Completing accurate progress notes and documentation

Essential Requirements

  • Current Driver's Licence
  • Reliable vehicle with insurance
  • Current NDIS Worker Screening Check
  • National Police Clearance
  • Working rights in Australia
  • NDIS Worker Orientation Module Certificate
  • Smartphone for rostering, communication and documentation
  • Minimum 1 year of relevant working experience and a qualification/certificate in a relevant field.

Most requirements can be obtained during our staff onboarding and induction phase once an application is successful
