We're looking for someone who genuinely cares.
At Dementia & Disability Home Services (DDHS), we believe exceptional support is built on genuine relationships, trust, and compassion. Every person we support has their own story, goals, and way of living, and we believe they deserve care that honours their individuality, empowers their choices, and helps them live with confidence.
Founded from lived experience, DDHS was created because we believe support should feel human. We know that the best outcomes come when people feel heard, respected, and genuinely cared for.
We're looking for compassionate Support Workers who share these values and want to make a meaningful difference every day.
About DDHS
DDHS is a Perth-based NDIS provider supporting people living with disability and dementia throughout the Perth Metropolitan (primarily Joondalup to Baldivis).
While we have a particular passion for supporting people living with dementia, we proudly support participants of all ages and disabilities. We believe in delivering personalised support that builds confidence, promotes independence, and creates meaningful connections for every participant and their family.
Our mission is simple:
To provide compassionate and respectful care that honours every person's story, fosters connections, and empowers people to live with joy and confidence.
About the Role
Every participant is unique, which means no two days are ever the same.

- The ability to work independently while being part of a supportive team
- A willingness to learn and continually improve
Whether you're an experienced Support Worker or someone looking to continue growing within the industry, we'd love to hear from you if you genuinely care about making a positive difference.
Essential Requirements
- Current Driver's Licence
- Reliable vehicle with insurance
- Current NDIS Worker Screening Check
- National Police Clearance
- Working rights in Australia
- NDIS Worker Orientation Module Certificate

- Minimum 1 year of relevant working experience and a qualification/certificate in a relevant field.
