Disability Support Worker (NDIS) – Melbourne

Faith Walk Support is a nurse-led NDIS provider delivering high-quality, person-centred care to participants with mental health conditions, psychosocial disabilities, intellectual disabilities, and complex needs. We are looking for reliable and compassionate Disability Support Workers to join our growing team.

Key Responsibilities:

  • Provide support with activities of daily living including personal care, grooming, and hygiene
  • Assist participants with community access, appointments, and social engagement
  • Support participants with mental health and behavioural needs
  • Assist with meal preparation, household tasks, and skill development
  • Follow individualised care plans and promote participant independence
  • Maintain accurate progress notes and documentation
  • Build positive, respectful relationships with participants and their families

Requirements:

  • Experience in disability support, aged care, or mental health
  • NDIS Worker Screening Check
  • Working With Children Check (preferred)
  • Current First Aid and CPR
  • Valid driver’s licence and access to a reliable vehicle
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
