Stream Services is seeking compassionate and experienced Lifestyle Support Workers to join our growing team in Clermont, QLD. This rewarding role supports people with disability to live independently, stay connected with their community, and achieve meaningful personal goals.

Ideally you will have experience in the disability sector and are looking to grow your career in a truly supportive, positive team environment with ongoing training and opportunities for development.

What You’ll Do

  • Support NDIS Participants with daily living activities, including personal care, meal preparation and household tasks
  • Provide social and recreational support to build confidence, independence and community participation.
  • Develop and follow individualised support plans aligned with Participant goals
  • Maintain accurate NDIS compliant documentation and reporting.
  • Work collaboratively with the wider team to deliver person-centred, high-quality care.

About You

We’d love to hear from you if you have:

  • Qualifications in disability support, aged care, community services – or equivalent experience as a carer.
  • Experience providing person-centred support to NDIS or aged care clients (preferred but not essential).
  • A warm, empathetic and professional approach when supporting others.
  • Strong communication skills, and the ability to build positive relationships.

  • Commitment to upholding the rights and dignity of people with disability
  • Ability to work independently and as part of a supportive team
  • Current First Aid and CPR Certificates (or willingness to obtain)
  • NDIS Worker Screening Check and Blue Card (or willingness to obtain)
  • Valid driver’s licence.
  • Full working rights in Australia
