Make a difference from the very first point of contact.
Every person's journey starts with the right support. In this role, you will be the first point of contact for people accessing Mission Australia services, ensuring they receive timely assessments, appropriate referrals, and person-centred support. You'll play a vital role in coordinating referrals, conducting intake assessments, managing client information, and connecting people with the services they need to achieve positive outcomes.
Working within a collaborative and supportive team, you'll combine strong administration, assessment and relationship management skills to ensure vulnerable clients receive responsive, high-quality support while maintaining accurate records and meeting compliance requirements.
Your key responsibilities will be to:
- Respond to and coordinate referrals from internal and external stakeholders
- Conduct intake and eligibility assessments to determine client suitability and support needs
- Complete client intake documentation, consent forms, assessments, surveys and applications when the service support coordinator is unavailable
- Provide information, brief interventions and referrals for clients requiring low to medium levels of support
- Develop individual support plans and allocate clients to appropriate to service support coordinators
- Refer clients to other suitable services
- Maintain accurate client records, case notes and documentation using client management systems
- Complete reports, statistics, assessments and data entry in line with organisational and funding requirements
- Monitor referral pathways and identify opportunities to improve referral and intake processes
- Build strong relationships with community organisations, government agencies and referral partners
- Participate in networking activities and promote effective collaboration across services
- Contribute to continuous improvement, quality assurance and program development initiatives.
- Provide guidance and share knowledge with team members to support best practice service delivery.
- Ensure compliance with Mission Australia policies, accreditation standards and the National Case Management Approach.
- Assist with outreach activities where required.
Requirements for success
- Experience in client intake, assessment, case coordination or community services
- Relevant qualifications in Community Services, Social Work, Human Services or a related field (desirable)
- Strong understanding of trauma-informed and person-centred practice
- Excellent communication, assessment and stakeholder engagement skills
- Experience maintaining accurate client records and preparing reports and statistics
- Strong organisational skills with the ability to manage competing priorities
- Experience using client management databases and Microsoft Office applications
- Ability to build effective relationships with internal teams and external service providers
