Home Care Worker – Silverchain Group (Australia)

Job Summary

Silverchain Group is seeking a compassionate Home Care Worker to support older adults and people living with disability or health conditions in their own homes. You will assist clients with daily living activities, domestic tasks, and social support while promoting independence, dignity, and overall wellbeing.

Key Responsibilities

  • Provide personal care, including assistance with hygiene, dressing, grooming, and mobility.
  • Support clients with daily living activities in line with individual care plans.
  • Perform light household duties, including cleaning, laundry, meal preparation, and grocery shopping.
  • Provide companionship and assist clients in accessing medical appointments and community activities.
  • Monitor client wellbeing and report any changes or concerns to the appropriate team members.
  • Maintain accurate care records and follow workplace health, safety, and infection control procedures.
  • Collaborate with nurses, allied health professionals, care coordinators, and other support staff.
  • Participate in training and ongoing professional development.

Qualifications

  • Certificate III in Individual Support (Ageing, Disability, or Home and Community Care) is desirable; training may be provided for entry-level candidates.
  • Current First Aid and CPR certification, or willingness to obtain.
  • Valid Australian Driver Licence and access to a reliable, insured vehicle.
  • National Police Check and other required clearances.
  • Strong communication, interpersonal, and customer service skills.
  • Compassionate, reliable, and able to work independently in the community.
