Health Navigator – Pathway to Good Health
Fixed Term, Part Time (0.6 FTE) opportunity until 30 June 2027

Our role is also supporting people to self-manage long-term health conditions, such as diabetes and mental health challenges, with multidisciplinary teams delivering community-based programs in a comfortable setting. We also support people, including older people and people with disability, to stay living at home longer and to live well in their community.
Our broad range of primary health services, care management and in home and community support services can be tailored to an individual’s needs and with major hubs in Box Hill and Eltham, these services can be delivered locally, in the Eastern and North Eastern suburbs of Melbourne.

Job Description
As the Health Navigator for the Pathway to Good Health program you will:
- Provide health assessments, triage and early intervention support for children and young people in Out of Home Care.
- Build trusted relationships with young people, carers and support networks, using a trauma-informed and child-centred approach.
- Coordinate referrals and connect clients with multidisciplinary health, wellbeing and specialist services.
- Advocate for children and young people to ensure their healthcare needs are identified and addressed.
- Deliver secondary consultation and guidance to carers, support staff and stakeholders supporting young people in care.
- Collaborate with Child Protection, paediatricians, allied health professionals and community services to achieve positive health outcomes.
- Maintain accurate client records, care plans and risk management processes in line with program and legislative requirements.
- Participate in outreach visits, multidisciplinary teamwork, professional development and continuous improvement activities.
Desired Skills and Experience
About You:
- Qualified Division 1 Nurse OR hold a relevant Allied Health qualification (Social Work, Occupational Therapy, Psychology, Speech Pathology, or similar).
- Current AHPRA registration, or eligibility for registration with the relevant professional body.
- Experience working with children and young people, particularly those experiencing vulnerability, trauma, or complex needs.
- Demonstrated ability to apply trauma-informed, holistic and strengths-based approaches in practice.
- Strong understanding of health systems, referral pathways, and community-based supports for children and families.
- Experience working collaboratively within multidisciplinary teams and alongside a range of stakeholders.
- Excellent relationship-building, communication, problem-solving, and advocacy skills.
- Ability to manage competing priorities, maintain accurate records, and confidently use Microsoft Office applications.
- Experience in paediatric, community health, public health, Child Protection, or related settings will be highly regarded.
- Current Victorian Driver's Licence and willingness to undertake community outreach as required.
