About the Role
Are you a passionate Social Worker looking to make a real impact in community care? We’re seeking an enthusiastic clinician to join our innovative team, helping patients receive high-quality, person-centred care from the comfort of their own homes. Join our team as a Grade 1 Social Worker on a full time, fixed term basis (1.0 EFT / 80 hours per fortnight) until July 2027, based across Kingston Centre (Heatherton) and Cranbourne sites.
In this role, you’ll provide best-practice, evidence-based care through face-to-face and/or Telehealth assessments and interventions for patients admitted to the service. You’ll work collaboratively with a supportive multidisciplinary team to ensure effective and compassionate care aligned with Monash Health’s Strategic Guiding Principles.
What will your day look like? You will:
Complete psychosocial assessments and provide evidence-based interventions for older adults receiving care in their own homes.
Support patients and families through counselling, advocacy, education and emotional support.
Collaborate with a multidisciplinary team to deliver coordinated, person-centred care.
Assist with complex discharge planning and connect patients with community, aged care and disability services.
Respond to psychosocial issues including grief, elder abuse, family violence and safeguarding concerns.
Provide care through home visits, with flexibility to use Telehealth or telephone where appropriate.
Participate in supervision, professional development and quality improvement activities.

About Us
Monash at Home - Aged and Rehabilitation Care (ARC) is a dynamic service that delivers care to patients within their own home. The team consists of Medical, Nursing, Allied Health, Pharmacy, Psychology and Administration staff. We provide an alternative to inpatient care, providing safe and high quality home based services for individuals with complex medical, functional and cognitive issues, who require comprehensive, medically led multidisciplinary assessment and intervention within the comfort of their own home. We are a 40 bed services that covers the Monash Health catchment. Operating out of two sites, Kingston Centre (Heatherton) and Cranbourne, and staff are required to work flexibly across both sites to meet patient needs.
Patients within the service receive daily nursing visits. Medical, allied health, neuropsychology, and pharmacy teams also provide face-to-face or Telehealth consultations, depending on individual needs. In addition, patients have access to 24-hour telephone support.
Our service is dedicated to delivering short-term (approximate length of stay is 2 weeks), high-quality, and compassionate care that’s safe, effective, and centred around each patient’s needs.
