Full job description
The Social Worker at Haven Care Australia plays a key role in supporting participants with disabilities, psychosocial challenges, complex care needs, mental health concerns, and social vulnerabilities to achieve improved wellbeing, independence, community participation, and quality of life.
The role involves working collaboratively with participants, families, guardians, support coordinators, healthcare professionals, and multidisciplinary teams to provide advocacy, psychosocial support, crisis intervention, case management, and linkage to appropriate services and supports.
The Social Worker will support Haven Care Australia’s commitment to delivering person-centred, trauma-informed, culturally sensitive, and high-quality supports in line with the principles of the NDIS Practice Standards and organisational values.
Key ResponsibilitiesParticipant Support & Advocacy
- Conduct psychosocial and environmental assessments for participants
- Support participants in achieving goals outlined in their NDIS plans
- Provide emotional support, advocacy, and practical assistance to participants and families
- Assist participants to access community resources, healthcare services, housing, mental health services, and other relevant supports
- Support participants during complex transitions including hospital discharges, SIL transitions, crisis accommodation, and change of providers
- Promote participant choice, control, dignity, and independence
Case Management & Coordination
- Work collaboratively with Support Coordinators, Behaviour Support Practitioners, Allied Health teams, hospitals, guardians, and family members
- Participate in case conferences, multidisciplinary meetings, and care planning discussions
- Assist with risk assessments, safeguarding concerns, and incident follow-up processes
- Support participants experiencing behavioural, emotional, social, or environmental challenges
- Assist in identifying risks related to homelessness, neglect, abuse, social isolation, or mental health deterioration
Documentation & Compliance
- Maintain accurate case notes, assessments, reports, and participant records
- Ensure documentation is completed in line with NDIS Practice Standards, organisational policies, and legislative requirements
- Contribute to report writing, care planning, and evidence gathering for participant reviews and funding reassessments
- Support compliance with incident management, mandatory reporting, and restrictive practice obligations where applicable
Community Engagement
- Promote social inclusion and community participation opportunities
- Support participants to build confidence, relationships, life skills, and independence
- Assist participants in engaging with education, employment, recreational, and community-based activities
- Foster positive relationships with external stakeholders and community services

Qualifications & Requirements
- Bachelor’s Degree in Social Work
- Eligibility for membership with the Australian Association of Social Workers (AASW) preferred
- Current National Police Check
- Working with Vulnerable People (WWVP) / Blue Card / Working With Children Check
- Current Driver’s Licence
- NDIS Worker Screening Clearance
- Current First Aid & CPR Certificate preferred
- Experience within disability, mental health, aged care, or community services sector desirable
