Summary:
The Support Coordinator plays a vital role in the social care sector, providing essential support to individuals and families in Geelong, Victoria. This position is designed for entry-level candidates who are passionate about making a difference in the community. The Support Coordinator will assist clients in navigating services and resources, ensuring they receive the necessary support to enhance their quality of life.
Responsibilities:
- Assist clients in identifying their needs and goals to develop personalised support plans.
- Coordinate and facilitate access to services and resources for clients.
- Maintain accurate and up-to-date records of client interactions and progress.
- Collaborate with other professionals and service providers to ensure comprehensive support.
- Provide information and guidance to clients and their families regarding available services.
- Monitor and evaluate the effectiveness of support plans, making adjustments as necessary.
- Participate in training and professional development opportunities to enhance skills and knowledge.
Qualifications:
