About the Role
Join the CO-ACT team at Hampstead Rehabilitation Centre and support older people to achieve the best possible outcomes through person-centred social work. You'll manage complex caseloads, work closely with multidisciplinary teams and help patients transition safely to the next stage of their care.
You're an experienced Social Worker who is passionate about supporting older people through compassionate, evidence-based care.

You're an experienced Social Worker who is passionate about supporting older people through compassionate, evidence-based care.
- Degree eligible for AASW full membership
- Experience managing complex health care caseloads and discharge planning
- Strong assessment, counselling and case management skills
- Ability to work collaboratively across multidisciplinary teams
- Experience providing supervision and clinical guidance
