About the Role:

You will coordinate and support the delivery of community-based aged care, disability and home support services using a Consumer Directed Care approach. Working as part of a multidisciplinary team, you will assist consumers to achieve their goals and remain living independently in their own homes by coordinating services, supporting staff, monitoring quality outcomes and collaborating with clients, families and community stakeholders.

Please note: Travel between sites is required as services are delivered across the region.

About You:

  • Experience coordinating or supervising community, aged care, disability or health services.
  • Strong leadership, communication and organisational skills with the ability to manage competing priorities.
  • Experience preparing reports, maintaining databases and using Microsoft Office; experience with online systems is desirable.
  • Ability to build positive relationships with clients, families and multidisciplinary teams while delivering person-centred care.
  • Current driver's licence and willingness to travel across the region as required.
