Full job description
- Deliver Care Without Boundaries: Leverage innovative Telehealth technology to provide dietary assessments and reviews, ensuring residents receive exceptional nutrition support no matter where they're located.
- Make a Meaningful Difference In Person: Conduct comprehensive dietary assessments within our care homes and develop personalised care plans that support residents' health, wellbeing, and quality of life.
- Lead and Empower Others: Mentor and support site-based Carer Therapists (AHAs), providing supervision, coaching, and guidance to ensure safe, effective, and resident-focused care delivery.
- Champion Nutrition Education: Empower residents, families, and care teams with practical nutrition advice and education that promotes healthier outcomes and informed decision-making.
- Collaborate with Experts: Work alongside a multidisciplinary team of healthcare professionals—including GPs, Nurse Practitioners, Physiotherapists, Wound Care Consultants, and Clinical Managers—to deliver holistic care for every resident.
- Support Quality and Compliance: Contribute to exceptional resident outcomes by upholding Aged Care Quality Standards and best-practice clinical and ethical guidelines.
- Grow Your Career: Stay at the forefront of dietetics by engaging in ongoing professional development, training, and learning opportunities.
- Accredited Practising Dietitian (APD) with a relevant tertiary qualification in Dietetics and current Dietitians Australia membership.
- Aged Care experience (ideally 3+ years) with a strong understanding of the unique nutritional needs of older adults.
- Exceptional clinical assessment and care planning skills, with the ability to develop personalised nutrition strategies that drive positive outcomes.
- Confidence in delivering care via Telehealth, using technology to connect with and support residents across multiple locations.
- Outstanding communication and relationship-building skills, enabling effective collaboration with residents, families, and multidisciplinary teams.
- A proactive and adaptable approach, with the ability to work autonomously while thriving in a collaborative team environment.
- A current Australian Driver's Licence, access to a reliable vehicle, and willingness to travel between care homes as required.
