This social worker role at Wollongong Hospital (Illawarrra Shoalhaven Local Health District / NSW Health) is a clinical, patient‑facing position focused on supporting patients and families with psychosocial issues linked to illness, injury, and hospitalisation. You will be responsible for organising safe discharge plans, linking patients to: community services, NDIS, aged care, housing or mental health supports.

The role also involves participating in the after-hours social work on call roster between Wollongong and Shellharbour hospitals.

Wollongong Hospital is the Illawarra Shoalhaven’s major referral and teaching hospital and has a bed-base count of more than 500. The campus, which incorporates the Illawarra Regional Cancer Care Centre, provides a comprehensive range of inpatient, outpatient and community-based services.
