Job Description

The Opportunity
We are seeking a Lived and Living Experience Worker – Chronic Conditions to join our supportive and professional multidisciplinary team, within an innovative health pilot supporting people living with chronic and complex conditions.

This is a fixed-term role until 30 June 2027, commencing at 0.3 FTE for the first five months and increasing to 0.4 FTE for the remaining seven months.

Responsibilities include but are not limited to;

  • Provide peer informed support to clients living with chronic and complex conditions

  • Use lived experience to build trust, hope and empowerment

  • Support clients to set goals and navigate health, social and community services

  • Assist with system navigation and appointment preparation

  • Identify barriers to care and support clients to address them

  • Work collaboratively within a multidisciplinary team

  • Contribute to co-design and service improvement activities

  • Support evaluation and outcome measures

Desired Skills and Experience

Skills & Experience

  • Lived or living experience of managing a chronic condition

  • Demonstrated ability to use lived experience constructively in a peer support context

  • Strong interpersonal and communication skills

  • Ability to support people navigating complex service systems

  • Ability to work collaboratively within a multidisciplinary team

  • Commitment to recovery-oriented and strengths-based approaches

  • Experience in community health, peer support or similar (desirable)

  • Experience participating in co-design or service improvement (desirable)

  • Certificate IV in Mental Health Peer Work or equivalent (desirable)
