About the role

Metropolitan Youth Health works on prevention, early intervention, and youth specialist health response measures regarding adverse childhood events and their impact on young people’s health. Programs focus on building and strengthening safe and positive relationships for all young people, with a special focus on vulnerable priority population groups.

Make a real difference in the lives of vulnerable young people! Deliver holistic health assessments, case management and psychosocial support, connect young people with vital services, and build strong community partnerships. Empower young people to take charge of their health, pregnancy, and parenting journeys while leading impactful programs that drive lasting outcomes.

Key duties of the role

• Facilitate the access of vulnerable young people to relevant primary health care solutions.
• Use knowledge, evidence and experience to achieve agreed patient care outcomes.
• Deliver case management services that empower young people to engage in their health care management.
• Identify, plan and coordinate the delivery of projects that provide a forum for young people to discuss issues relating to health, pregnancy, and parenting and improve their ability to self-manage these issues.

Skills and Experience

• Appropriate Degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers.
• Provision of a high standard of assessment, case planning and health care services.
• Planning, facilitation, evaluation and delivery of group-based projects for young people.
• Knowledge of adolescent psycho-social development, mental health and youth health issues.
• Demonstrated ability to develop and deliver training and development programs.
