Full job description

Woody Point Medical Centre is a large, multidisciplinary private medical practice with nine General Practitioners and a dedicated team of health professionals. We are committed to delivering high-quality, patient-centered and preventative healthcare. As our services continue to expand and diversify, we have observed increasing socio-cultural health needs within our community, as well as a growing number of patients presenting with chronic and complex conditions.

We recognize that effective prevention, early detection and sustained health education are essential to reducing the long-term health burden for these patients. In response to our practice expansion, community feedback and the increasing demand for structured preventive-health programs, we are seeking a qualified Health Promotion Practitioner to strengthen our capacity in delivering coordinated health promotion and diagnostic-engagement initiatives.

This is a full-time, ongoing professional role working closely with GPs, practice nurses and the broader clinical team. The Health Promotion Practitioner will design, deliver and evaluate evidence-based health promotion activities, patient-education programs and disease-prevention initiatives that are fully integrated within the medical practice, with the goal of improving both individual and population-level health outcomes.

Key Responsibilities

· Develop, implement, and evaluate evidence-based health promotion and preventive-health programs within the medical practice and broader community.

· Coordinate patient engagement in early detection and screening pathways (e.g., chronic disease checks, immunization awareness, lifestyle risk assessments, cancer screening participation).

· Deliver individual and group health education sessions on lifestyle risk factors, chronic disease prevention, behavior change strategies, and general health literacy.

· Create and maintain patient education materials aligned with GP management plans, preventive health priorities, and national screening recommendations.

· Collect, interpret and report health program data to support clinical decision-making, service improvement and quality assurance requirements.

· Work collaboratively with GPs, practice nurses, allied health clinicians, and community organizations to implement wellness and prevention initiatives.

· Support diagnostic engagement by promoting awareness of recommended screening and early-detection services, and by assisting patients to navigate referral pathways.

· Plan, coordinate and evaluate targeted health campaigns addressing key population-health priorities (e.g., cardiovascular risk, diabetes prevention, respiratory health, women’s and men’s health).

· Contribute to clinical governance activities including program monitoring, outcomes reporting, and alignment with practice accreditation standards.

· Maintain professional conduct, confidentiality, and compliance with all relevant health regulations, guidelines, and organizational policies.

Essential Requirements

· Bachelor degree or higher in Public Health, Health Promotion, Community Health, Health Science, or a closely related field.

· Demonstrated professional experience in health promotion, preventive health programs, patient education, or diagnostic-support roles in primary care, community health, or medical practice settings.

· Strong understanding of population-health principles, behavior-change strategies, and preventive-health frameworks.

· Excellent interpersonal and communication skills with the ability to deliver patient education and engage diverse groups.

· Proficiency in data collection, analysis, outcome reporting and program evaluation.

· Ability to work both independently and collaboratively within a multidisciplinary clinical team.

· Current Working With Children Check and National Police Check (or ability to obtain).

Desirable

· Minimum 3 years’ experience in general practice, medical centres, or primary health networks.

· Experience developing preventive-health campaigns or coordinating screening participation initiatives.

· Multilingual communication skills and experience supporting culturally diverse communities.

What We Offer

· Competitive salary based on qualifications and experience.

· Supportive, dynamic and collaborative practice environment.

· Ongoing opportunities for professional development and training.

· The opportunity to play a key role in improving patient health outcomes through preventive care, health education, and diagnostic engagement.
